Which insurances do you accept?
We currently accept the following insurances:

How can I schedule an appointment?
You can schedule an appointment by calling our office at (772) 446-3403 during business hours or by using our online appointment request form on our website.

What should I bring to my appointment?
Please bring your insurance card, a valid ID, and any relevant medical records or medications. New patients should arrive early
Email or fax the new patient packet to our office prior to your appointment.

How can I refill my prescriptions?
You can request prescription refills through our patient portal or by calling our office. Please allow 24-48 hours for your request to be processed.

What is your cancellation policy?
We understand that circumstances may arise that require you to cancel or reschedule your appointment. Please notify us at least 24 hours in advance to avoid a late cancellation fee. Late cancellations (less than 24 hours notice) and missed appointments may be subject to a fee of $50. This fee helps us maintain appointment availability for all patients.
